[Bug 2127844] [NEW] Bluetooth drops connections frequently

Public bug reported:

Bluetooth connections to any/all bluetooth devices have become very
unstable - all connections will fail at the same time. The dmesg logs
show

[94692.756106] Bluetooth: hci0: Hardware error 0x0c
[94692.770097] Bluetooth: hci0: Retrieving Intel exception info failed (-16)

when it happens. The latest version of bluez, 5.79-2ubuntu1.3, made the
problem a lot worse, but it seems to also be present on 5.79-2ubuntu1.
